Use this form to submit a soil sample to the WVU Soil Testing lab located at the Davis College of Agriculture, Forestry and Design in Morgantown. Fill out the form as completely as possible and attach it (with a rubber band) to a sandwich bag containing 1 cup of soil (about half full). Mail it (them) in a large envelope or box to:
